  • Control type
  • Part number
  • Ship to address
  • Bill to address
  • PO Number

If your request concerns a control or individual part currently under warranty, you will also need the control serial number or PO the individual part was purchased under. A purchase order will be required for tracking purposes only. No charges will be applied to a warranty purchase order with the exception of special shipping and handling charges for counter to counter or door to door deliveries, or damage to the part not covered under warranty.

If your request concerns a new control, motor or drive system under warranty contact your OEM.

Note: There will be a special handling charge of $200.00, plus shipping assessed for all orders which require special delivery services (door to door, counter to counter).
